Our Medical Home Support Center works with your family and your medical home (wherever your child receives his/her medical care) to coordinate a family–focused, community–based, culturally–sensitive care plan.

- An Approach to providing high quality, cost effective health care services.
- A Partnership between families and health care professionals.
- A Means for children and families to receive care from professionals they know and trust.
- A Map through the maze of services, school programs, legislation and information providers that families are faced with everyday.
What Do We Do?
At the Northwest Regional Medical Home Support Center, we are here to help caregivers of children with special health care needs. We provide:
- Care Coordination
- Family Support
- Advocacy
- Title V Funds
- Benefits Coordination
Working with caregivers, we help create customized care plans and bring together the resources that families with special needs kids depend upon. We are also here to parnter with pediatric care providers in meeting the diverse needs of caregivers and families of children and youth with special health care needs.
