What Can We Do For You?
Our Medical Home Support Center provides a wide variety of resources and support services including care coordination, family advocacy, information about support programs and technical assistance. Payment for certain goods and services is subject to availability of funds and is determined on an individual basis depending on health insurance coverage and family income. Payment for these services must also relate to your child’s diagnosis.

Thank you for your interest in the Children and Youth with Special Health Care Needs Program (CYSHCN). The following links make up the "registration packet" which includes an application, a release of medical information, a respite application and a screener. In order to register, please print out, complete and return the following forms to us either by fax or by mail. Your registration packet must include:
- Completed application
- Signed and dated release of information
- Completed respite application
- Completed screener
In order to download the forms above, you must have the Adobe Reader program. If you do not have Adobe Reader, you may download it for free by clicking on the Adobe Reader button on the right.
- Proof of Income (Copies of last 4 pay stubs or most recent income tax return)
- Proof of Expenses (copies of receipts)
- Insurance Information (copy of insurance card – both front and back)
If you have any questions regarding these forms, please feel free to call at any time. These will be used to determine eligibility for both respite services as well as extended services (i.e., durable medical equipment, specialized nutritional formulas and prescription medicines) not covered by insurance in the event those would be needed.
