Who is Eligible?
Families of children and youth age 0 to 21 who have or are at increased risk for a chronic physical, developmental, behavioral or emotional condition and who also require health and related services of a type or amount beyond that generally required by their peers.
All families of eligible children and youth, regardless of income, will receive care coordination services in partnership with their child’s medical home. Family and community referrals, direct services, advocacy and linkages with parent support are also provided. A limited number of families will receive respite and camp funds. Uninsured or underinsured families, who fall within the Title V Extended Service Funds Eligibility Guidelines can also benefit from payment for extended services (i.e. durable medical equipment, prescriptions, specialty visits, therapies and special nutritional formulas).
