What is A Medical Home?
The medical home is a nationwide initiative overseen by the American Academy of Pediatrics that seeks to build partnerships between families and their pediatric care providers. A medical home does not refer to a place, but rather to an approach to providing cost effective, high quality medical care. Children and families who have a medical home receive the care they need from pediatric care providers who they know and trust. Caregivers and pediatric care providers work together as partners to find and determine all of the medical and non-medical services that their children and families need to meet their potential. According the American Academy of Pediatrics, a medical home should provide care that is:
