What is the Northwest Regional Medical Home Support Center?

Think of a Regional Medical Home Support Center (RMHSC) as a map through the maze of services, school programs, bills and information needs providers and families are faced with every day. RMHSC's are located across the nation. Our center is one of five here to serve the five regions of Connecticut. At the Northwest RMHSC, our team of professionals is here to provide information and support to pediatric providers and families caring for children and youth with special health care needs (CYSHCN) in the Northwest region of Connecticut. Most importantly, we help foster and develop the partnerships between families and their medical homes.
